Prepare Your Vendor Packet
Please have the following documents ready before completing the registration form. Upload clear, complete, and current copies with all applicable pages included.
Documents Required
Certificate of Insurance - COI
Provide a current Certificate of Insurance showing your company’s applicable insurance coverage. Ask your insurance agent to list the following as the certificate holder:

Check that your company name, policy information, and coverage dates are readable before uploading the certificate.
Completed W-9
Upload your completed and signed Form W-9. Make sure the business or taxpayer name and other information are accurate and consistent with the entity that will provide services and receive payment.
Workers’ Compensation Documentation
Upload your current Workers’ Compensation insurance documentation. If you are submitting an applicable exemption instead of a policy, upload the exemption documentation and identify it in the form for review.
If the Workers’ Compensation coverage is included on your COI, you may upload that same certificate in the Workers’ Compensation field.
Tell Us About Your Business
Along with your documents, provide your primary contact information, trade or specialty, service areas, and any relevant qualifications. You may also include licenses, certifications, a company overview, or examples of your work.
The more clearly you describe your capabilities, the easier it is for our team to understand where your business may fit an upcoming project.

What Happens After You Submit?
We review your information. Our team reviews your company details, services, and submitted documents.
We follow up when needed. We may contact you for clarification, updated documents, or additional information relevant to a project.
We discuss potential opportunities. If your capabilities align with our needs, we will reach out to discuss the scope and next steps.
Submitting a vendor packet does not guarantee approval or an award of work. Project requirements, pricing, scheduling, and any required agreements are addressed separately before work begins.

Please provide updated documents when insurance policies renew or when your company, contact, or tax information changes. Keeping your packet current helps us review opportunities using accurate information.
